(i386_io_port_add): Fix TSS access and locking accordingly.
(i386_io_port_remove): Likewise.

* i386/i386/pcb.c (switch_ktss): Now takes next thread as parameter
for TSS switch to be task-based. Fix all callers.

(machine_task_terminate): ... here. New function.
