Add the nominal_fn field to struct ps_fmt_spec, which will be used to decide
whether values are `unexciting'.
Add entries for proc_stat_list_for_each and proc_stat_list_spec_nominal.
Rename ps_fmt_squash to ps_fmt_squash_flags.
