(refresh_dir):
Add PRESERVE_ENTRY parameter; all callers changed.
(ftpfs_refresh_node):
Record ENOENT entries.
(ftpfs_dir_lookup):
Handle "" lookups like ".".
Set E's name timestamp for noent entries.
(ftpfs_dir_null_lookup):
New function.
